A current physician’s order for each medication to be administered.
Medication in the original, labeled packaging.
If this medication is not currently stored at Pride, please email a doctor's script to lguerrero@eclcofnj.org by deadline listed above.

We will bring all emergency medications already stored at PRIDE with us to the event.
